Passports

The Gray County Register of Deeds office serves as an Acceptance Agent for the U.S. Department of State for passport execution. 

Some requirements are:

  • The form DS-11, “Passport Application” must be filled out in black ink only and signed before an authorized Acceptance Agent.
  • Proof of citizenship: this is a certified copy from the state of your birth certificate or citizenship papers.
  • Proof of identity: this is usually your driver’s license. It can be any ID with your picture on it.
  • Must submit 1 color photograph taken by a professional photographer of the applicant alone, 2x2 inches in size. You can get these taken at Walmart or Walgreen’s.

Payment Type

All payments must be made by check or money order. Each person’s application must have a separate check or money order. Applications can get separated while at the Passport Agency, so they may not come in the mail at the same time even if applied for at the same time.

Special Passports/Requirements

You can now get a passport card for land and sea only to Canada, Mexico, the Caribbean, and Bermuda for a separate fee. You can apply for a passport book and card, or one or the other.

There are special requirements for children under age 16 applying for a passport. Please inquire with an Acceptance Agent for details.
